1、本人自愿报名参加2020“慕思杯”厚街半程马拉松赛及一切相关活动(以下统称“比赛”)；
2、本人全面理解并同意遵守组委会及协办机构(以下统称“组办方”)所制订的各项规程、规则、规定、要求及采取的措施；
3、本人已在医院做好体检，身体健康，为参赛做好充分准备，并愿意按照有关要求接受药物检查；
4、本人已将参赛适宜告知紧急联系人和其他亲属，并得到了他们的同意；
5、本人全面理解参赛可能出现的风险，且已准备必要的防范措施；本人愿意承担比赛期间发生的自身意外风险责任，且同意组办方对于非组办方原因造成的伤害、死亡或其他任何形式的损失不承担任何形式的赔偿；
6、本人同意接受组办方在比赛期间提供的现场急救性质的医务治疗，但在医院救治等发生的相关费用由本人自理；
7、本人授权组办方及指定媒体无偿使用本人的肖像、姓名、声音和其它个人资料用于比赛的组织和推广；
8、本人承诺以自己的名义报名并参赛，绝不将报名后获得的号码布和计时芯片以任何方式转让给他人;
9、本人同意在比赛前和比赛期间不得损害、更改及遮盖马拉松官方号码布;
10、本人同意向组办方提供有效的身份证件和资料用于核实本人的身份，并同意承担因身份证件和资料不实所产生的全部责任;
11、本人同意组办方以本人为被保险人投保人身意外险，具体内容已从保险说明书中知晓，本人均予以认可;
12、本人保证按照参赛半程马拉松项目的要求提供三甲（含）以上医院的心电图体检报告或以往半程马拉松的参赛证明，并在领取装备时提供心电图检查报告或以往参赛证明和报名信息表，如不提供，本人愿意承担取消比赛资格的责任，报名费不予退还。
13、本人同意在注册报名时填写正确的报名信息，不得有误。理解并同意遵守赛事每项只能申报一项，多报，均不退报名费的规定。
14、本人或法定代理人已认真阅读并全面理解以上内容，且对上述所有内容予以确认并承担相应的法律责任。

1、I myself voluntarily register for the 2020 Houjie Half Marathon and all activities in accordance with this event (hereafter referred to as “competition”);
2、I understand comprehensively and agree with all regulations, rules, requirements and measures instituted by the Organizing Committee and event co-organizers (hereafter referred to as “organizers”);
3、I am in good health condition in which I fully prepared for the competition, and I am willing to accept the doping test required by the organizers;
4、I have already informed my emergency contact person and relatives about attending the competition, and have already got there permit.
5、I understand comprehensively all potential risks in the competition, and has already prepared precautionary measures based on personal health; I am willing to undertake the full responsibility of all personal emergency risks which occur during the competition, and agree that the organizers do not undertake any compensations and responsibilities of injury, death, or any other forms of loss, which is caused by participants themselves;
6、I agree to accept the first aid and emergent medical treatment provided by the organizers during the competition, and I shall pay the expenditures by myself in accordance with medical treatment incurred in hospital;
7、I authorize the organizers and designated events media to free use my portrait, name, voice and other personal information in the organization and promotion of the competition;
8、I promise that I shall register to the competition based on all my true individual information, and will not transfer my bib number and timingchip to any others;
9、I agree to do not deliberately damage, change or mask my bib number before and during the competition;
10、I agree to provide valid personal identity documents and information to organizers for personal status confirmation, and undertake full liabilities due to the false individual documents and information provided;
11、I accept that personal accident insurance provided by the organizers, and all detailed clauses have been recognized and approved from the insurance instruction book;
12、I certify that according to the requirements of the competing half marathon project to provide formal medical institutions of medical report (including ecg examination), and to provide a medical report when receive equipment (including ecg examination) and the registration information form, if you don't provide, I am willing to accept the responsibility of disqualified, registration fees will not be returned
13、I agree to fill in the correct registration information during registration, may not be wrong. Understand and agree to abide by the tournament every can declare a sign, not the provisions of the registration fee.
14、I or my legal representative has fully understood this declaration with duly acceptance and confirmation, and bear corresponding legal responsibilities.
